Decision Tree end points include:

• Recheck INR in 4 weeks
• Recheck INR in 2 weeks
• Adjust dose 10-20% and recheck INR in 1-2 weeks
• Physician reevaluation
• Omit 1 dose, decrease regular dosing 10-20% and recheck INR 1-2 weeks
• Omit 2 doses, recheck INR in 2 days, physician revaluation
