- What is the target INR range?
- Is current INR either 1.7-2.0 or 3.0-3.5
- Recheck INR in 1 week:
- Indicate current INR:
- Is current INR either 2.3-2.5 or 3.5-4.0
Decision Tree end points include:
- Recheck INR in 4 weeks
- Recheck INR in 2 weeks
- Adjust dose 10-20% and recheck INR in 1-2 weeks
- Physician reevaluation
- Omit 1 dose, decrease regular dosing 10-20% and recheck INR 1-2 weeks
- Omit 2 doses, recheck INR in 2 days, physician revaluation
- Repeat INR STAT, physician reevaluation
