      Males: >= 36mL/kg
      Females: >= 32 mL/kg
      Arterial O2 saturation >= 92%
Group B
      Splenomegaly
Group C
      Platelet count > 400,000/microL
      WBC count > 12,000/microL in the absence of fever or infection
      Leukocyte alkaline phosphatase (LAP) score > 100 in the absence of fever or infection
      B12 > 900 pg/mL or serum unbound B12-binding capacity > 2200 pg/mL
